The Department of Homeless Services and Housing clinical team works closely with people experiencing and transitioning out of homelessness to determine the type of housing and level of clinical services that best meet their unique needs.
To make these assessments, HSH clinicians conduct evaluations for reasonable accommodations, in-home caregiving, Enhanced Care Management (ECM), and medical or behavioral support services. Finally, clinicians assess program participants to determine whether their current housing is the model that best meets their needs. For example, someone living in Enriched Residential Care (ERC) who has achieved housing stability may be able to successfully transition to more independent living in another type of permanent housing.
HSH clinical staff provide direct medical services to people in interim housing, Permanent Supportive Housing, and Enriched Residential Care settings until they have reached housing stability and are connected with appropriate outpatient care.
Direct medical services include, but are not limited to:
- Management for hypertension, diabetes, and congestive heart failure
- Wound care
- Occupational therapy
- Medication management support
HSH clinical staff provide care management services to program participants who live with complex health conditions to support their overall wellbeing and journey to long-term housing stability.
Clinical staff work closely with contracted case managers to ensure they have the resources to support these participants. Program participants are assessed to ensure they receive the support they need to remain successfully housed and prevent returns to homelessness.
HSH provides training and technical assistance to contracted case managers and facility operators to equip them with the knowledge, skills, and resource connections to administer high-quality care to program participants that meets them where they are.
HSH clinical staff partner with the service providers operating interim, Permanent Supportive Housing, and Enriched Residential Care sites to provide trauma-informed, low-barrier care, ensure clinical issues are safely addressed, and actively connect participants with the resources they need to regain personal stability.
