HFH Transition to HSH: What Providers Need to Know

The formal transition from Housing for Health (HFH) to the Department of Homeless Services and Housing (HSH) took place on January 1, 2026.

Temporary disruptions to tools and systems were necessary to ensure a successful and secure transition. Below you will find a summary of what to expect and how to prepare.

About This Transition

On January 1, 2026, Housing for Health (HFH) formally transitioned to the LA County Department of Homeless Services and Housing (HSH). This transition coincides with the New Year holiday and may create temporary inconvenience for staff and providers.

Temporary disruptions to tools and systems are necessary to ensure a successful and secure transition. Updates will continue to be shared as systems come back online and normal operations resume under the new department.

ICMS Snapshot Pause

Generation and distribution of the ICMS Snapshot paused from December 24, 2025 through January 1, 2026. Distribution resumed the week of January 5, 2026, though some delays were possible as systems were fully restored.

Microsoft Teams Access

External guest access to private Microsoft Teams Channels was temporarily disrupted during the transition. HFH/HSH staff worked with ICMS Providers to re-establish individual channel access under the new department beginning January 1st, extending into the week of January 5, 2026.

Email and System Access

HFH staff received new HSH email addresses. Short delays in regaining access to email, SharePoint, Teams, CHAMP, HMIS, and other systems were possible during the transition period. New email access was available to HSH team members as of January 1st.

What to Expect: System Disruptions

The following tools and systems experienced temporary disruptions during the HFH to HSH transition. Review each area and take action where needed to maintain communication and service continuity.

Important: Not all information carries over 1:1 when systems migrate. Anticipate potential delays and disruptions to email and system communications during the beginning of January 2026.

ICMS Snapshot

  • Snapshot generation and distribution paused December 24, 2025 through January 1, 2026
  • Last Snapshot of 2025 was generated on the evening of December 23, 2025
  • Distribution expected to resume the week of January 5, 2026
  • Some delays possible as systems are fully restored

Microsoft Teams

  • External guest access to private Teams Channels temporarily disrupted during transition
  • HFH/HSH staff worked with ICMS Providers to re-establish individual access under the new department
  • Process began January 1st and may have extended into the week of January 5, 2026

Email Access

  • HFH staff received new HSH email addresses as of January 1st
  • Some staff may have experienced short delays in regaining full email access
  • Most HFH staff had auto-replies set up with their new HSH email address
  • Coordinate a communication strategy with at least one key HFH contact for this transition

Other Systems

Short delays in access were also possible for the following systems during the transition period:

  • SharePoint
  • CHAMP
  • HMIS
  • Other department platforms

What Providers Should Do

To minimize disruption to services and communication during this transition, ICMS Providers should take the following steps.

Re-Establish Teams Access

Work with your assigned HFH/HSH staff contact to re-establish individual access to Microsoft Teams channels under the new department. This process began January 1st.

Update Your Key Contacts

Coordinate a communication strategy with at least one key HFH contact before the transition. Confirm their new HSH email address so communication is not interrupted during the changeover period.
