ICMS Assessment Tools
ICMS case managers are required to complete two quarterly assessments for enrolled participants. Both tools are available for download below.

5×5 Risk Stratification Tool
Also known as: HFH Risk Stratification Tool (based on VAT)
A structured assessment that evaluates participant functioning across five domains: Physical Health, Mental Health, Substance Use, Life Skills, and Activities of Daily Living. Each domain is scored 1 to 5, where higher scores indicate greater risk of adverse outcomes.
Completed at enrollment and reassessed every 90 days. Results directly inform care planning and service intensity.
Housing Acuity Index (HAI)
Also known as: Connecticut Supportive Housing Assessment — Acuity Index
An assessment that evaluates housing stability, income and benefits, health engagement, and access to supportive services for permanently housed participants. Each item is scored 0 to 3, where higher scores indicate greater self-sufficiency.
Required quarterly for participants who are permanently housed. Completed alongside the 5×5 reassessment.
5×5 Documentation Tool
A standardized documentation review tool that evaluates whether case notes accurately reflect participant needs, interventions, and progress across the five domains assessed in the 5×5 Risk Stratification Tool. It promotes consistent, complete, and participant-centered documentation that supports quality care and program compliance.
Used as a quality assurance and coaching tool to strengthen documentation practices and ensure alignment between assessments, service delivery, and case notes.
