Participant Success Stories
Housing is a milestone worth celebrating. These stories highlight the journeys of participants who have moved into stable housing through LA County’s PH ICMS program. Case managers are encouraged to share participant stories and photos to recognize housing milestones across the system of care.

Tanieksha Jonescooper has been with us in our SRAP Pre-Match program since the end of March 2025. She had been experiencing homelessness for a couple of years and was able to access interim housing at Hilda L. Solis in January 2025. She was matched to Abbey Apartments in September and has officially moved into her PSH unit.
Ms. Jonescooper has gone from unemployed and unhoused to employed at the Crocker Care Campus and housed at Abbey Apartments for PSH. Her journey is a testament to what is possible for participants when they are empowered with economic opportunity and PRE-MATCH housing navigation services through ICMS — a story the entire system of care can be proud of.
Previously Featured Participants
Whether a person has been living on the streets for years or only recently fell into homelessness, their journey is unique and often complex. Homelessness does not have to mean hopelessness. With help, many people have been able to make their way home, heal, and thrive. Get to know their stories.
Share a Participant Story
Note: Do not share the submission email address directly with participants or ask participants to contact this address themselves. All submissions must be coordinated by the case manager or program staff.
Case managers are encouraged to submit participant photos and stories that celebrate housing milestones. Submissions help recognize the impact of ICMS services and highlight participant progress across the system of care.
Get the Release Form Signed
Have the participant sign the DHS Photo Release Form before submitting any photos or story content.
Prepare the Photo and Story
Prepare the participant’s photo or photos and a brief written story about their journey to housing.
Submit to HSH
Email the signed release form, photo(s), and story to:
