Prevent Homelessness Promote Health (PH²)
PH² is a joint program between the LA County Departments of Mental Health (DMH) and Health Services Housing for Health. The program works with adults and families countywide to address risk factors and build skills that support the maintenance of permanent and stable housing.
Homelessness is preventable. In order to end homelessness, we must support people’s overall health in their homes.

About the PH² Program
Prevent Homelessness Promote Health (PH²) is a joint program between the LA County Departments of Mental Health and Health Services Housing for Health. PH² works with adults and families countywide to address risk factors and build skills that support the maintenance of permanent and stable housing.
Services consist of brief interventions and other appropriate treatment modalities provided by an integrated team of mental health clinicians and physical health care staff and providers.
Note: PH² is an advanced outreach option for participants at risk of eviction who are not engaged in mental health services and reside in PSH buildings without onsite HSSP. If your building has HSSP onsite, contact HSSP directly rather than initiating a PH² referral.
Mental Health Services (DMH)
PH² provides mental health stabilization and clinical support through the LA County Department of Mental Health. Services focus on individuals whose mental health needs are putting their housing stability at risk.
Medical and Physical Health Services (DHS)
PH² provides medical and physical health case management through LA County Health Services Housing for Health. Services focus on individuals whose physical health needs are creating barriers to stable housing.
Services Offered and Eligibility
Single adults and families who are at risk of eviction and meet the eligibility criteria below are eligible for PH² services. The program provides both standard and as-available services based on individual need.
Eligibility Criteria: Mental health need, resident of Los Angeles County, currently in permanent supportive housing or a licensed residential facility, and at risk of eviction.
Standard Services
- Mental health stabilization — if currently linked to MHS, ensuring the individual is actively participating at an appropriate level of services
- Identify and link to appropriate housing
- Building communication and problem-solving skills
- Crisis and conflict resolution
- Harm reduction education
As Available
- RN assessment for home safety modifications
- RN support for chronic disease management, medication adherence, and health literacy
- RN coordination support for higher levels of care
Submit a PH² Referral
PH² accepts referrals for participants at risk of eviction who have an identified mental health or medical need. Use the correct referral contact based on the nature of the participant’s primary need.
Note: Do not use PH² if your PSH building has HSSP onsite. Contact HSSP directly for participants in buildings with onsite mental health services.
Mental Health Need Referrals
Send referrals for participants with a primary mental health need to the DMH PH² team.
Medical or Physical Health Need Referrals
Send referrals for participants with a primary medical or physical health need to the DHS medical case management team.
